Impacted Canines: Diagnosis, Challenges, and Surgical Treatment
Permanent canines play a fundamental role: they are a cornerstone of your smile aesthetically, but above all they have a critical functional role in your bite. By guiding jaw movement during lateral motion, they protect the entire dentition.
Yet, after wisdom teeth, canines are the teeth most frequently blocked (impacted) in the jawbone or under the gum, unable to erupt naturally.
Why Does a Canine Remain Impacted?
Impaction of a canine generally occurs when the tooth cannot find its ideal eruption path. The most common causes include:
- A lack of space on the dental arch.
- An abnormal migration path (an initial orientation anomaly of the tooth).
- A baby tooth that did not fall out at the right time.
- An anatomical obstacle or the presence of tissue along the eruption path.
This situation most often affects the upper jaw (often on the palate side) and can be one-sided or affect both sides.
The Risks of an Untreated Impacted Canine
Leaving an impacted canine untreated exposes you to several medium- and long-term complications:
- Damage to neighboring roots: The pressure exerted by the blocked canine can damage the roots of the adjacent incisors.
- Resorption or abnormal retention of the baby tooth.
- Cystic complications: Rare but possible formation of cysts around the impacted crown.
- Misalignment and loss of harmony of the dental arch.
Multidisciplinary Care: Surgery and Orthodontics
Treatment of impacted canines almost always relies on close collaboration between the maxillofacial surgeon and the orthodontist. The main goal is to save the natural tooth, free it, and guide it into its ideal position on the arch.
1. The Pre-Operative Assessment (3D Imaging)
Careful planning is essential. Using three-dimensional imaging (CBCT / Cone Beam), Dr Yousefpour analyzes with millimeter precision the exact position of the canine and its proximity to delicate structures and neighboring roots, in order to choose the gentlest and safest surgical approach.
2. The Surgical Procedure (Exposure and Bonding)
The procedure consists of freeing the crown of the canine held within the bone:
- A small incision allows delicate access to the tooth.
- The bone covering the crown is carefully exposed.
- An orthodontic traction device (a small attachment or chain) is bonded directly onto the surface of the canine.
- The gum is then repositioned and sutured (often with resorbable sutures), leaving the wire or attachment exposed.
3. Orthodontic Traction
A few days after the procedure, the orthodontist can begin applying very light traction forces to progressively guide the canine along its ideal path until it reaches its natural position.
What Happens After Surgery?
- Anesthesia: The procedure can be performed under simple local anesthesia, or combined with sedation (or general anesthesia) depending on the case and desired comfort.
- Discomfort: Mild pain or moderate swelling may occur for a few days. Appropriate pain relievers will systematically be prescribed.
- Recommendations: Rigorous hygiene, antiseptic mouth rinses, and a lukewarm/cold diet for the first few days promote optimal healing.

Surgery to expose an impacted canine is a common procedure, but like any medical act, it requires a full understanding of the issues, benefits, and potential risks involved.

Risks and Potential Side Effects
Although we do everything possible to minimize post-operative effects, it is important to be informed of the risks inherent to any oral surgery:
- Common risks: Swelling of the operated area, pain, or a bruise are possible and generally resolve within a few days.
- Specific risks: Although rare, complications such as accidental damage to the roots of neighboring teeth (notably the incisors) due to their proximity, or discomfort from scarring in the gum area, can occur.
- Eruption failure: Despite a precise surgical technique and appropriate orthodontic traction, the canine can sometimes present with ankylosis (the tooth becomes fused to the bone), preventing it from being brought into place. This can lead to failure of the conservative treatment, in which case extraction of the tooth is then indicated.
